We work with healthcare and community partners to bring health screenings, healthcare support, insurance enrollment help, and local resources directly into neighborhoods. Medical services and screenings are provided by partner organizations and licensed healthcare providers. The Health Collaborative does not provide medical care. Our role is to connect people to services, coordinate support, and help community members find affordable health coverage.
Pathways to Health was created to bring health-related services closer to where people live, work, worship, and gather. Many individuals face barriers that make it difficult to access preventive care, understand health coverage, complete referrals, or follow through with appointments.
This is more than a one-day event or health fair. By bringing together healthcare partners, community organizations, health plans, and trusted neighborhood leaders, we create welcoming opportunities for people to receive screenings, ask questions, connect to resources, and get follow-up support when additional care or services are needed.
The Health Collaborative is a San Antonio-based nonprofit organization with more than 29 years of experience advancing community health through collaboration, facilitation, data-informed planning, and cross-sector partnerships.
The Health Collaborative organizes Pathways to Health by bringing community and healthcare partners together, coordinating the events, and providing care coordination and health insurance navigation services that connect people to the care and resources they need.
We partner with healthcare organizations, licensed providers, health plans, public health agencies, community organizations, universities, volunteers, and service providers to bring health resources into local neighborhoods.
